1. BEHAVIORAL ASSESSMENT

 

Please call us to schedule a free behavioral assessment and tour. This will take about 30-40 minutes and is required prior to booking. We typically do behavioral assessments 12:30pm-3:30pm, Mon-Sat. All dogs must pass our behavioral test in order to be granted entrance into The Madison.  We have the right decline entrance into our facility if certain requirements are not met.  Behavioral assessments are by appointment only.

2. VACCINATION & HEALTH REQUIREMENTS

 

Please bring a copy of your dog's vaccination records upon arriving for the behavioral assessment.  This ensures the safety of all the dogs in our facility, as well as our staff.  No exceptions - dogs without current vaccination records will not be permitted in the facility. Proof of all vaccinations listed below is required.

REQUIRED VACCINATIONS & PREVENTION:

• Bordetella (required every 6 months)

• Rabies

• DHPP (Distemper/Hepatitus/Parvovirus/Parainfluenza)

• Regular flea & tick treatment

HEALTH REQUIREMENTS:

• Dogs must be spayed/neutered

 

Please Note: Puppies under 6 months are not required to be spayed/neutered, however, they will not be eligible for pack play.

We reserve the right to remove any dog at any time from pack play for the safety and well being of all of our dogs and staff.

3. REGISTRATION FORM & CLIENT WAIVER

Once your dog has passed the behavioral assessment test, please fill out the registration form below and bring it to your first appointment.
